Improving Outcomes Through Healthy Living

 

We provide personalized treatment for patients ages 6-18 who have been diagnosed with obesity, hypertension or high cholesterol.  Our providers can help your child lower their blood pressure, lower their blood sugar and reduce their cholesterol through healthy living.  When we focus on a child's cardiovascular health, we can improve health outcomes over their lifespan.

Obesity is a complicated diagnosis that requires specialized education and treatment. Our program is individualized to patient and family needs. All patients are seen every week for the first month of our program. Patients are then seen every other week for months 2-6.  In our visits we discuss topics such as:

        nutrition                                           physical activity                                   sleep

        emotional eating                              stress regulation                                  screen time

Convenient for Families

Because of the time required to make healthy lifestyle changes, we have found that telehealth visits can be just as effective and much more convenient for families.

A patient's initial visit is scheduled in the morning and followup visits are scheduled in the afternoon to accommodate work and school schedules.

Getting Started

Because we are a telehealth clinic, we require a referrals for all patients. A referral includes a copy of a note with a physical exam performed in the last 90 days and any pertinent labwork. Contact your primary care provider to send us a referral.

Dr Sarah Babcock Homer CPNP-PC


Dr Homer has 10 years of pediatric obesity experience in Utah County.  She earned her bachelors from Brigham Young University and her doctorate from the University of Utah.  She is certified by the National Association of Pediatric Nurse Practitioners and is a member of the Obesity Medicine Association and the American Academy of Pediatrics.  She enjoys spending time with her family, running and sewing.

Dr Gabriela Ramirez FNP-BC

Dr Ramirez has 17 years of experience as a healthcare professional.  She earned her doctorate from Weber State University and holds national certification as a Family Nurse Practitioner. Gabriela is also a member of the American Nurses Association and Sigma Theta Tau (Honor Society of Nursing).  Gabriela finds joy in exploring the world through travel and immersing herself in the beauty of the great outdoors.
